Nassau to Cuba is extremely easy. Air Cubanna flies one flight a day to Havana, around 3PM, and one from Havanna to Nassau usually in the morning. The plane is definitely vintage, but fortunately it's a short flight, about an hour.
There is an agent for Havanatour, the state travel agency, based in Nassau. His name is Michael Larrow and he works for the Greycliffe Hotel, you should be able to find the phone # online. He will arrange for the flights, visas and can get you a very good deal on accomodations.
He usually meets you at the Nassau airport to give you the tickets, visas and travel vouchers. He can be a little flakey and not that easy to get ahold of, but as long as you leave enough time between connections you'll be fine.

Although I am a longtime Brilander, I highly recommend visiting Cuba, it is a truly unique and wonderful place. Have a great trip!
